If a friend or family member has asked you to complete a survey for him or her, you're in the right place!

Just a few brief instructions, before you get started

The survey you are about to complete is an anonymous survey. To maximize the accuracy of this tool, the person you are evaluating will never see your responses, but instead, will receive a report that combines your responses with others who have evaluated him or her.

For each survey item, please indicate the extent to which the statement is true of the person you are evaluating. It is important that you consider carefully not just the person’s recent behaviors, but instead, think back over the past year or so that you’ve known this person. For best results, please think about each question for at least 20 seconds before responding. Also, if you do not know the answer to any item, please click on “I Don’t Know.”
